Facilities in StrataEMR represent your physical practice locations. Each facility has its own dashboard, schedule, task list, and letterhead. When multiple locations are active, staff and patients are associated with specific facilities, and all reporting and billing flows through the correct location automatically. Navigate to Facilities setup: Menu Bar > Settings > Facilities.
Access: Viewing facility settings requires Therapy Director permissions or above. Adding new facilities and making address or letterhead changes requires coordination with StrataPT — these are not fully self-serve.

Adding a New Facility

To add a new facility, select Add New Facility from the Selected Facility dropdown. Enter the facility’s name, address, phone, fax, email, and other contact details.
Adding a facility generates a work order for StrataPT staff to complete. Creating the facility record in Setup initiates the process, but StrataPT completes the configuration — including assigning staff to the facility to add them to the schedule. Contact your account manager with details about which staff should be assigned to the new location.
Facility information must be correct before go-live. An incorrect or missing address can cause Medicare EDI enrollment delays and claim processing issues.

Facility Letterhead

Each facility has a custom letterhead that appears on faxes, clinical documentation, and patient forms. The letterhead auto-builds from the facility’s contact settings — phone, fax, website, and name — and displays dynamically based on the patient’s treatment location. Patients always see the letterhead for the facility where they are being treated. If you would like to change your letterhead image, provide your StrataPT account manager with the best quality version of the image you want to use and they will apply it. A style control file is maintained behind the scenes that defines the header, fonts, and print formatting.

Switching Between Facilities

If your practice has multiple locations, use the facility dropdown in the top-left corner of the dashboard to switch between them. Selecting a different facility updates the Waiting Room, Tasks, Analytics, and Schedule to show information specific to that location. Staff with the View Only Own Facilities Schedules permission modifier will only see facilities they are explicitly assigned to in the dropdown.

Assigning Staff to a Facility

When a new staff member is added, StrataPT assigns them to the appropriate facility on your behalf. If you need a staff member added to a new or different facility, message your account manager via the StrataEMR messaging platform with the staff member’s name and the facility they should be assigned to. Staff who are not assigned to a facility will not appear in patient demographics dropdowns for that location.

Address Changes and Insurance Notifications

Changing a facility’s address is not just an internal update — it has billing and credentialing implications. Required steps when a facility address changes:
  • Notify all insurance companies you are contracted with in advance
  • For Medicare, submit a new 855B application with a copy of the signed lease and updated malpractice certificate showing the new address. StrataPT will place a credentialing hold while the Medicare application is in process.
  • For commercial payers, requirements vary — contact each payer to confirm their update process. An improperly updated address can result in claims processing as out-of-network.
